About the Massachusetts State Home Care Program
The Massachusetts State Home Care Program provides care management, care coordination, advocacy, education, and in-home and community support services for adults 60+, people with disabilities, and people under age 60 living with Alzheimer’s disease or related dementia.
Home care services, including the Frail Elder Waiver, are available to Massachusetts residents and help adults aged 60+, people with disabilities, and people with early-onset Alzheimer’s disease or related dementia successfully age in place.
Home care services are also available to individuals under the age of 60 if the person has early-onset Alzheimer’s disease or related dementia to successfully age in place.
Services are available based on assessed need, and the cost for those services is determined based on income. Services provided by the Home Care Program may include the following:

Benefits of Home Care
Advocacy
- Care coordination by trained, professional care managers and nurses.
- Support & professional assistance through the ASAP network.
- Help with navigating a vast array of care needs.

Care Management
- Knowledgeable in age-appropriate topics, assessing and problem resolution.
- Development and management of person-centered comprehensive care plans, as well as regular ongoing person-centered assessments to address any new needs.
- Engagement with community and service providers.
- Referrals & connection to service providers and other community resources/partners.
Service Provider Network
- Adheres to programmatic standards.
- Regularly monitored & audited.
- Staff vetted through CORI and OIG Exclusion List.
- Contracts with state accepted providers for specific services.
Service Rates
- Leverages rate setting established by MassHealth for specific services.
- Set contracted and negotiated service specific rates with the ASAP.
- Less expensive than private pay.
- No minimum service plan required.
Eligibility
- Eligibility for the Home Care Program is determined by need and based on age, residence, and ability to carry out daily tasks.
- Applicants for the Home Care Program must be living at home within Massachusetts.
Margaret is 78 years old and lives alone. She has dementia and arthritis that makes it difficult to get around. She wants to stay in her own home, but her daughter worries about her safety. Margaret struggles to remember her medications and keep up with housework. Through the Home Care Program, she receives personal care help three times a week and visits from a care manager who coordinates her services. She also attends a day program twice a week for social activities. The program allows Margaret to remain independent in her home while staying safe. Her daughter has peace of mind knowing she's getting the support she needs.
Cost
Cost of the Massachusetts State Home Care services is based on income. Cost share guidelines can be found here.
